Saint francis hospital is seeking an experienced full-time aco behavioral health specialist to join our team!
The behavioral health specialist (bhs)/msw is integral to population health management, care coordination and behavioral health care through collaboration with the physician, care management team, and other healthcare partnersThe bhs/msw works with the attributed patient population identified as high or rising risk via stratification and referral, and those with behavioral health conditions impacting health outcomesThis role focuses on psychosocial factors and social determinants of health impacting outcomes of individuals with comprehensive health needsEssential functions include assessment, behavioral health interventions, connections to social and community services, facilitation and coordination across the continuum including skilled nursing facilities, and follow up to promote quality, cost effective outcomes aligned with the triple aim.
Masters in social work (msw) required, lcsw preferred
Two years of medical social work experience with the medicare population required
Demonstrates competency in brief problem-focused and evidence-based interventions such as motivational interviewing, behavioral activation, problem solving treatment, cognitive behavioral therapy including dialectical behavior therapy skills
Possesses working knowledge of available crisis, drug and alcohol , and other community resources to address a wide range of social determinates that may be impeding outcomes
Five years of experience required in community-based or home care social work
Knowledge of discharge and transition of care planning across the continuum required
Proven ability to build relationships with external partners/skilled nursing facilities, collaborate with snf teams and effectively drive appropriate/timely discharge planning and smooth transitions of care aligned with length of stay guidelines while improving outcomes
Strong leadership ability and excellent interpersonal, oral and written communication skills
Ability to work independently and remotely and as part of inter-professional team, make independent decisions and demonstrate flexibility, personal integrity and ability to work effectively with patients, colleagues and partners.
Ability to work with patients and families with diverse opinions, religious beliefs, cultural ideas and valuesCommitment to actively collaborate in a patient-centered care model that supports and honors individual choices.
Must have basic computer literacy, working knowledge of microsoft office and clinical database software
Valid driver`s license and ability to travel to patient homes and/or provider sites in delaware as needed
