We are seeking a dynamic director of care management to be the driving force behind the most critical aspect of lumata health’s mission; ensuring patients at risk of vision loss from chronic eye diseases are educated, engaged, and adherent to their care plansThe director of care management oversees all aspects of the care management program, including operations, patient communication development, policy guidance and enforcement, and managing care coordination personnelThis position reports directly to the president.
You will rely on your experience and exceptional interpersonal skills to build a world-class program and team to ensure lumata health is the leader in chronic care managementThe successful applicant is compassionate, knowledgeable about healthcare, and has experience leading a care coordination team.

Participate as a key member of the leadership team
Manage the day-to-day operations of the care management program
Hire, train and supervise care coordinators and enrollment specialists
Ensure enrollment specialists are meeting enrollment goals
Oversee patient satisfaction measurement program
Manage the reimbursement parameters around the care coordination program to ensure revenue is maximized by both lumata health and its clients.
Implement analytics-driven barrier identification and elimination to ensure care coordinators are appropriately assessing and addressing barriers to care
Promote continuous improvement; recommending people, process and tools/software changes to meet or exceed department goals and add value to our customers/patients
Keep up-to-date on the latest patient/customer support technologies and industry trends to aid in our scaling efforts
Ensure clinical education amongst the care coordination team
Implement analytics-driven barrier identification and elimination
Drive world-class engagement and patient retention rates by training care coordinators to the proven communication style of motivational interviewing
At least 5 years` experience leading a care management or healthcare-related service team
Demonstrated ability to lead and build programs from the ground up
Excellent verbal and written communications skills.
Demonstrated ability to be productive in a remote working environment
Strong analytical thinking and ability to handle multiple tasks concurrently.
Excellent customer service.
Compassion and empathy.
Experience creating new processes and modifying existing processes to meet business needs
Experience with a crm or omni-channel service platform preferred
Experience hiring and developing talent across multiple locations
Proven track record of driving measurable efficiency results
Demonstrated ability to meet goals in a rapidly changing environment
Computer skills and proficiency in microsoft office.
Lpn, rn, bsn, or a ba/bs in social work, certified ophthalmic technician (cot) or related field.
Experience working at a tech-enabled healthcare startup
At least 3 years` experience in care management or clinical practices
