Full time positon with benefits
Conduct in-home assessments, eligibility mds-hc and completion of plans of care, provide information on louisiana medicaid long term care programs, provide community referrals.
Provide hcbs program choice counseling to requestors
Explain long term care programs to applicants (rights and responsibilities)
Complete needs-based assessment
Develop person-centered plan of care
Department of health and hospitals (dhh)
Office of aging and adult services
Bureau of health services financing (bhsf)
Office of citizens with developmental disabilities (ocdd)
Medicaid home and community based waivers
Understanding of healthcare in areas related to elderly and disabled adults
Education, training, and experience requirements:
Bachelor`s degree in health or human services field, social work, or related discipline, or registered nurse degree
Minimum of 2 years of health related social and/or human services experience
Minimum of 1 year experience working with older adjults or persons with disabilities
Valid louisiana driver`s license and current auto insurance.
Has no supervisory responsibilities.
Manages own workload.
Ability to problem solve without supervisory approval.
Evaluates and selects solutions from established operating procedures.
Exchanges information and ideas effectively.
Supports project management functions that align with client expectations and business needs.
Schedules project team meetings/calls.
Maintains project documentation and records.
Gathers project status updates.
Monitors project plan progress and adherence to deadlines.
Responds to project requests.
Performs other duties as assigned.
Complies with all policies and standards.
