Care4kids - care4kids is wisconsin`s foster care medical home and is a voluntary medicaid benefit that provides comprehensive health care coordination services to 3,000 children who were placed in out-of-home care in southeastern wisconsinThe program supports the unique health needs of these children and aims to improve health care outcomes by strengthening the quality, access, and timeliness of care utilizing a `medical home` model approach for each child who enrolls in the programCare4kids provides children with access to primary care physicians who are trained in the needs of children in out-of-home care, and also creates a team of professionals who coordinate care for the child.

Assists children`s community health plan (cchp) members with complex and chronic conditions utilizing problem solving techniques, referral to appropriate community resources, and/or referral to internal case managementThe outreach coordinator will work closely with the clinical team of nurses and social workers to assist with decision making.
Bachelors degree and 2 years of experience in health promotion, health advocacy, health education or community outreach.
Knowledge of the medicaid population and benefits preferred.
Completes telephonic, home, office or community based visits to support the healthcare and psychosocial needs of members and caregivers.
Requires a valid driver`s license, acceptable driving record, personal auto liability insurance coverage and the ability to be insured by the chhs auto insurance carrier.
