The clinical case manager is a licensed clinical social worker positionThis role partners with a registered nurse case manager to utilize a collaborative process of assessment, planning, and advocacy to meet a member’s benefit plan and/or health needsResponsibilities include researching available options and resources for members with financial, transportation, safety and housing needsPopulation spans the entire lifecycleAll member contact is done telephonically or via electronic exchangeThis position works to empower members to make positive lifestyle and medical choices and works as a team with other clinical staff to assist members with community resources for medical, financial and behavioral healthRequires an lsw or msw with unrestricted active license.
Lsw, lmsw, lcsw or lisw required
Complies with all state requirements in the state where job duties are performed
2+ years utilizing community resource expertise and alternate funding arrangements available to plan members when services are not available under the benefits program and/or providing psychosocial interventions through resource identification, and/or assisting with securing community resources and such as transportation, etc.
Standard hours: 8:30am-5:00pm est during training, 8:00-4:30pm or 8:30-5:00pm est thereafter.

2+ years experience in medical social work or
Masters degree licensed social worker
