Full job description
The care coordinator collaborates with mental health case managers, substance use providers, intellectual and developmental program service coordinators, primary care physician offices, and other internal and external service providers to promote coordinated care for persons and families in servicesThis position is based primarily from home and conducts most work through phone calls and tele-video sessionsAt the discretion of the program director or care coordination supervisor, may include office-based assignments.
The care coordinator utilizes the bluebonnet trails community services (btcs) electronic health record to monitor key health indicators and other dashboard information, such as referrals, missed appointments, crisis episodes, and moreThe care coordinator links individuals and families to healthcare providers, supports follow-up with internal and external providers, conducts routine assessments and surveys, recommends necessary interventions, and facilitates recovery planning.
The care coordinator provides culturally responsive and trauma-informed servicesThe position works within and promotes an integrated holistic model for healthcare at btcs, which is a certified community behavioral health clinic.
The care coordinator is connected to a core clinical team to serve approximately 250-300 peopleThe clinical team may serve more than one county and include more than one btcs programTherefore, this position requires an aptitude for researching and connecting with a variety of local resources across a multi-county areaIndividuals served by the care coordinator may have routine or intensive psychiatric, substance use, developmental or co-occurring needs.
This position reports directly to the care coordination supervisor.
City remote; clinic if needed
Credentials minimum qualifications: a bachelor’s degree or higher with a major in social, behavioral, or human service field including, but not limited to: psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human development, gerontology, educational psychology, education, or criminal justiceMinimum of 6-months case management / service coordination experienceMust have a valid texas driver`s license, meet bluebonnet trails community services driving requirements, and maintain approved driving status throughout employmentMust be willing to work a flexible work schedule to accommodate the needs of individuals and/or their familiesMust have skills and experience using integrated computer database programs to access files and document treatment services and the ability to organize and prioritize work assignments.
Preferred qualifications: master’s degree and/or clinical license in psychology, education, social work or a related human service field; one-year work or practicum/clinical experience in intensive case management and/or psychosocial rehabilitative services; bilingual in asl and/or spanish; experience in providing concurrent documentation within an electronic database preferred.
Salary amount $17.63 - 19.63/hr
