Ensures coordination of services and continuity of care of participantsDevelops and establishes intensive outreach strategies to maintain program capacityMonitors delivery of services and ensures continuous quality improvement in relation to program goals and standardsCoordinates and assists families to access appropriate supports during times of crisisWorks closely with other programs and service providers in the community to increase service access and ensures adherence to service agreement.
Bachelor’s degree in counseling, social work, psychology or related social services area from an accredited university or college.
Bilingual english/spanish preferred.
Two years of experience in community behavioral health setting.
Availability to work flexible schedule to accommodate families’ needs.
Basic computer skillsValid il driving license.
Licensed use of a car, current car insurance and driving record acceptable to agency insurance carrier.
