The american heart association has an excellent opportunity for a **quality improvement manager,** **atherosclerotic cardiovascular disease (ascvd)** based out of our national center office this position can be remote/home based!
This individual will be responsible for the implementation of quality programs for improving care and treatment of patients with ascvd in the assigned health care systemsResponsible for onboarding health care clients in the use of the aha outpatient platform with a focus on ascvd and risk factor related measures, engaging sites in an educational learning cohort, and driving systems to improved patient outcomesResponsible for partnering with a variety of clients such as primary care providers, general cardiologists, and their care teams in hospitals, clinics, or quality improvement organization to implement specific quality tactics (blood pressure control, cholesterol control, diabetes treatment, healthy lifestyle changes and lipid management)May also be responsible for identifying and implementing quality workflow enhancements for improving care and treatment.
+ provide consultation, direction, and collaboratively support health systems through learning cohorts on hospital and clinic barriers associated with cvd to deliver better care and outcomesFacilitate learning cohort meetings in coordination with volunteers and staff partners.
+ lead project management including budgets, timelines, activity plans and communication with team members and stakeholdersServe as point person for ongoing resources needed by hospitals and clinics to sustain knowledge (via online education, articles, etc.)In coordination with staff partners and volunteers, establish sustainability plan for all components of funding and programming.
+ implement systematic efforts in healthcare settings to identify and manage patients who would benefit from initiating or intensifying therapies using clinical decision support tools and/or patient decision aids as part of shared decision-makingWork with in-hospital clinical care teams to address barriers to guideline adoption and enhance clinical workflows for improved patient outcomes utilizing get with the guidelines for project data collection needs.
+ analyze results of training, education and kpi data to continually evaluate next steps and needs in the defined marketEnsure that issues related to project data (within get with the guidelines and other project reporting needs) are addressed appropriately and in a timely manner.
+ communicate regularly with project director and manager to assure tasks are completed according to project/program timelineWork with national staff partners through ongoing meetings to ensure program deliverables are achieved.
+ bachelor’s degree or equivalent work experience
+ experience working in healthcare administration, clinical treatment, or healthcare/medical patient access environment
+ experience in clinical healthcare or in a public health-related organization: experience may substitute in part for educational requirement with 1.5 years of work experience for 1 year of higher education
+ experience with quality improvement, program development and systems improvement.
+ project management/implementation experience
+ ability to influence performance without direct line accountability
+ skilled in working with clinical audiences to impact change management related to patient outcomes
+ strong verbal, written, and presentation communications skills
+ ability to function independently with minimal supervision
+ ability to work in a team environment and interact with all levels of american heart association staff, volunteers, and the public
+ comprehensive microsoft office suite skills including word, excel, powerpoint and outlook
+ ability to travel up to 15% local and overnight
**preferred skills: **
+ experience with smartsheet
+ basic knowledge of microsoft dynamics
+ proficiency with virtual meeting platforms (exZoom, gotowebinar)
